GastritaGASTRITA & ULCERUL

 

Gastrita este denumirea pentru un grup de afectiuni caracterizate de inflamatia mucoasei gastrice. Cauzele aparitiei gastritei pot fi:

(a) infectioase: infectii bacteriene, fungice, virale (CMV, HSV) sau  parazitoze;

(b) non-infectioase:

  • probleme alimentare (mancaruri prea grase sau prea picante, exces cofeina din cafea, ciocolata, ceai chinezesc)
  • stresul social cronic
  • reactii imune si autoimune (gastrita tip A)
  • reactii reactive (reflux biliar, in special dupa colecistotecmie, sau secretii pancreatice),
  • expunerea la tratamente orin radiatii,
  • expunerea la substante exogene: alccol, fumat, formaldehida, suplimente cu fier sau potasiu, medicamente medicamente antiinflamatoare nesteroidiene (ibuprofen), aspirina etc

Gastrita tip B cuprinde:  gastrita predominant antrala, gastrita asociata infectiei cu H. pylori, gastrita generata de factori exogeni. Tipul B de gastrita reprezinta forma cea mai comuna de gastrita cronica. La pacientii tineri, tipul B de gastrita afecteaza in principal antrul, in timp ce la pacientii mai in varsta este afectat intreg stomacul. Aceasta tranzitie se estimeaza ca are loc pe parcursul a 15-20 ani. Incidenta gastritei cronice, in majoritate de tip B, creste cu varsta, atingand 78% dintre pacientii peste 50 ani si virtual 100% din pacientii peste 70 ani.

Cea mai frecventa cauza a gastritelor cronice (cca 80-90%) o constituie infectia cu Helicobacter pylori, despre care puteti citi suplimentar aiciDaca infectia cu H. pylori nu este total eradicata, atunci ea poate progresa lent si poate conduce la gastrita atrofica multifocala, atrofie gastrica si metaplazie gastrica.

Gastrita netratata, in special cea infectioasa generata de Helicobacter pylori, este principala cauza de aparitie a ulcerelor gastric si duodenal si creste riscul de a face cancer gastric de 3-6 ori.
