MIOCARDITA

 

Miocardita (sau cardiomiopatia inflamatorie) este o conditie cardiaca caracterizata de inflamatia miocardului. Acesta este  este structural si functional anormal in absenta bolii arteriale coronariene, a hipertensiunii arteriale, a valvulopatiei, a bolii congenitale cardiace. Miocardita poate (sau nu) include necroza tesutului cardiac, si de obicei este asociata cu cardiomiopatia dilatata.

Inflamatia specifica miocarditei se datoreaza:

  • infectiei virale:  coxsakie, CMV,  adenovirus, parvovirus B19, HHV-6, HCV etc – cauza principala in Europa si SUA;
  • infectiei bacteriene (asociata imunosupresiei):  Borrelia burgdorferi,  Brucella, Corynebacterium, Haemophilus influenzae, Actinomyces etc;
  • infectiei  fungice: Aspergillus, Candida  etc;
  • parazitozelor:  ascarizi, schistosoma, Taenia solium, Trichinella spirali etc;
  • reactiei autoimune asciata infectiilor streptoccocice si virale Cox B – acesti microbi contin proteine curegiuni similare cu miosina specifica miocardului;
  • reactiei autoimune din boli sistemice:  SLE, scleroderma, sarcoidoza,  vasculita sistemic, granulomatoza  Wegener’s, boala  Kawasaki  etc
  • reactiei la anumite medicamente: etanol, antracicline, antipsihotice etc
  • reactiei la toxine (metale si minerale toxice, monoxid de carbon etc)
  • expunerii la radiatii, socuri electrice etc

Simptomele miocarditei cuprind:  dureri de inima (“junghiuri”), insuficienta cardica (edem, lipsa de aer), palpitatii, febra (in perioada infectioasa) etc, mergand pana la moartea subita la tineri (miocardita  in 20% din cazuri)

Diagnosticul corect de miocardita se pune pe baza biopsiei, dar acest lucru se practica extrem de  rar, din cauza riscurilor existente. In mod uzual, diagnosticul de miocardita este un diagnostic de excludere, suspicionat pe baza valorilor ECG, C-RP, ESR, IgM (anti-viral) etc
