LITIAZA RENALA

 

Litiaza renala este o afectiune caracterizata prin prezenta calculilor la nivelul rinichilor sau ureterei (canalul ce leaga rinichiul de vezica urinara), vezicii urinare sau ureterului (canalul ce leaga vezica de meatul urinar). In cca 90% din cazuri, calculii au marimea unui fir de nisip si se pot elimina cu usurinta, fara simptome, de catre sistemul urinar. In restul situatiilor, miscarea calculilor provoaca dureri intense (colica nefretica).

Litiaza renala este mai frecventa la barbati decat la femei:  cca 10% dintre barbati si 5% dintre femei vor avea pietre la rinichi in cursul vietii.  In general litiaza renala este o suferinta specifica persoanelor peste 30 de ani.

Substantele care intra in componenta calculilor renali sunt:

  • minerale:

- calciu (sub forma de oxalat de calciu, fosfat de calciu sau urat de calciu)  prezent in cca 75% din pietrele la rinichi;

- magneziu in cca 15% din pietrele la rinichi, sub forma de struvita (fosfati de amoniu si magneziu), caracteristice infectiilor urinare bacteriene cronice; mai frecvente la femei

- fosfor, prezent in struvita si litiaza din fosfat de calciu

  •  acizi organici:

- acidul oxalic prezent in oxalatii de calciu

- acidul uric prezent in cca 6% din pietrele tip urat de calciu, dar important deoarece 25% din persoanele cu acest tip de pietre vor avea, de asemenea, guta

  • aminoacizi:  cisteina prezenta in 2% dintre pietrele la rinichi.

Datorita complicatiilor si durerilor provocate de eliminarea pietrelor, este importanta detectia precoce a litiazei renale si tratamentul ei neinvaziv. Acest lucru este importnat si datorita faptului ca 50% din persoanele cu litiaza renala vor avea din nou calculi renali in termen de 10 ani dupa prima criza.
